Court Forms for Name and Gender Marker Changes

If you are an adult without a felony conviction, you should use the Self-represented Adult Name and Gender Change Packet
If you are an adult with a felony conviction, you should use the Self-represented Felony Conviction Name and Gender Change Packet
If you are seeking to change the name of a minor, you should use the Self-represented Minor Name and Gender Change Packet.
If you are seeking to only change your gender marker (and not your name), you should use the Self-Represented Gender Change Only Packet.
Once your name and gender marker are changed, you can use the Updating Your Identity Documents guide to have your birth certificate, social security card and BMV id match your correct name and gender marker.
